The revised Dependent Student Eligibility for Additional Unsubsidized Stafford Loans Checklist may be used by schools to help in documenting a dependent student's eligibility for additional loan assistance. Financial aid staff may download and copy this checklist form as necessary. - AES School Late Disbursement Checklist/Notice

In November 2006, the U.S. Department of Education (ED) provided a revised version of the matrix published in August 2006. The revisions include additional information based upon questions that were submitted in response to the initial document and a new address to use when submitting information-only partial disability discharge packages to the Conditional Disability Discharge (CDD) Unit for joint Consolidation loans, co-made PLUS loans, and loans with an endorser. - Partial Disability Discharge: Joint Consolidations, PLUS Co-Borrowers, and Endorsers (84 KB PDF)
This document was developed as a joint effort between ED/Federal Student Aid and the National Council of Higher Education Loan Programs (NCHELP). Lenders and servicers may refer to this document for guidance for handling discharge applications received for spousal consolidations, co-made PLUS, and PLUS loans with disabled endorsers.
